The SUD Ambulatory Follow-up dataset displays percentages of Medicaid discharges for members 6-64 years of age who were hospitalized in a SUD inpatient setting that were followed by a lower level SUD service within 14 days of discharge.

The MH Engagement in Care dataset displays percentages of Medicaid discharges for members 6-64 years of age who were hospitalized in an inpatient setting with a primary diagnosis of mental illness that were followed by two or more outpatient visits, intensive outpatient encounters or partial hospitalizations for mental health treatment within 30 days of discharge.

This dataset contains the number of discharged ACT participants at admission to ACT and at discharge from ACT within a set of outcome indicators such as living situation, employment and risk behaviors. The data are aggregated by New York State region, year of discharge (past 5 years), AOT status at admission, and by behavioral health diagnosis at admission.

This dataset provides the number of case closings in each month by outcome of case. Abuse and neglect case outcomes are determined by the outcome of the allegation(s) within the case.

This dataset shows the number of Staff Exclusion List (SEL) checks performed by month. Provider agencies under the Justice Center’s jurisdiction, as well as other providers identified in statute, are required to check the SEL before hiring someone who will have regular and substantial contact with an individual with special needs.

This provides information on the location, contact persons, services provided and capacity of New York State Commission for the Blind (NYSCB) Comprehensive Service Contractors.
